This form is for Blue Devils members, coaches and team managers to submit an incident report. This can be for any item related to team and club activity. A copy of this form will be sent immediately to the Club President and Vice President and will remain confidential to the club committee and/or the governing bodies, including the KBA and Basketball Victoria, at all times.
